From the outset, the resident is front and centre of our thinking and developing a personalised care plan that will enhance their quality of life is our top priority.
We begin before the resident even arrives at our door, making sure we complete a comprehensive preassessment that involves a discussion with the resident themselves (if medically possible) about what their priorities are for their care.
Once we have spent time with the resident and their family, we can then work out an individual care plan that answers everyone’s needs. The communication doesn’t stop once the resident is living at Homelands; we actively encourage and initiate dialogue with the residents and their families so they get the optimum level of care.
We have regular residents meetings that allow discussion of any areas for change or improvement, anonymous biannual questionnaires whose contents are made publically available and a suggestions box for those spur of the moment ideas.
Each resident is allocated a named nurse who has responsibility for assessing, planning and evaluating care for that resident, they also have a care assistant who acts as a key worker and is responsible for the social care needs of the resident.
